Cancellation Policy
Please fill out the short form, share your ideal time for a consultation or to schedule therapy. This information will be sent to our office manager for scheduling confirmation. We will reach out via email or text to confirm appointment and send new client documents for you to fill out. Additionally, our office manager will confirm if we are in network with your insurance (if you plan to use insurance) and confirm your cost of therapy if you are paying out of pocket.
